Registry Innovations in Collaborative Care

Registry Innovations in Collaborative Care (CoCM) will highlight two organizations’ process implementing and using a registry with two different electronic health records and registry tools. The panelists will describe how their organizations began the process of developing a population-based tool for CoCM and implemented the tool with their CoCM team at a patient and program level.
